چکیده

Although in most cases of haemolytic disease of the newborn practically all the bilirubin in the serum is of the indirect reacting type, a quarter of a century ago Still (1927) and Hawksley and Lightwood (1934) described cases in which the serum direct reacting bilirubin was raised and signs of obstructive jaundice appeared. This condition came to be known as the 'inspissated bile syndrome' (Hsia, Patterson, Allen, Diamond and Gellis, 1952). In 1959 Oppe and Valaes reported a systematic study of this condition. They confirmed that the findings of hepatosplenomegaly (Lathe, 1955), gross anaemia and hyperbilirubinaemia, including an elevated conjugated fraction (Cornblath, Kramer and Kelly, 1955; Stempfel, Broman, Escard6 and Zetterstrom, 1956), were present at birth. They also noted that the jaundice in these babies took on a greenish hue and that a tendency to bleed sometimes developed. Recent papers (Billing and Lathe, 1958; Zuelzer and Brown, 1%1b) have shown that the 'indirect' fraction is made up of free bilirubin and bilirubin monoglucuronide, both of which are largely formed at extrahepatic sites. The 'direct' fraction represents bilirubin diglucuronide which is probably formed only in the liver. TABLE I

Obstructive jaundice in haemolytic disease of the newborn treated with magnesium sulphate.

Obstructive jaundice seldom follows haemolytic disease of the newborn. It may occur at the height of the haemolytic process or not until some weeks later. In the latter instance, an evanescent phase of haemolytic jaundice may occur after birth with a recurrence of icterus after a few weeks, and then biliary obstruction. The degree of biliary obstruction is usually only partial but it may be com...

Decrease of serum cholesterol ester in haemolytic disease of the newborn.

The conception of the pathological entity of the three manifestations of haemolytic disease of the newborn: hydrops foetalis, icterus gravis and congenital haemolytic anaemia has been confirmed by the discovery of the Rhesus factor. Moreover, this discovery has allowed us finally to abandon any sharp clinical distinction between the two latter forms of the disease. This is in accord with the fa...

Chronic non-haemolytic jaundice with conjugated bilirubin in the serum and normal liver histology: a case study.

Recently our knowledge of bilirubin metabolism has increased considerably. Perhaps the most stimulating discovery has been that bilirubin is excreted in the bile as a glucuronide (Billing, Cole and Lathe, 1957; Schmid, 1957). This has made it possible to classify some rare types of chronic jaundice more definitely. One of these, chronic non-haemolytic familial jaundice, was described many years...

A Genetic Study of Neonatal Obstructive Jaundice.

Clarification of the problem of jaundice in the neonatal period began with the discovery of foetomaternal rhesus incompatibility as the usual cause of haemolytic disease of the newborn. After this, the group of infants in whom the jaundice is of obstructive type, with pale stools and bile in the urine, received more attention, and it was soon realized that not all such infants suffered from atr...

Urinary potassium excretion in newborns with and without icterus neonatorum.

The precise mechanism underlying the common 'physiological' jaundice of the newborn remains uncertain (Goldbloom and Gottlieb, 1929; Billing and Lathe, 1956; Lathe and Walker, 1957; Nelson, 1964). This study was based upon a previous suggestion that increased haemolytic activity operates in some cases, and that an increased urinary potassium concentration is one expression of this, continuing a...
